The goal of the Saint Mary School Nurse's Office is to ensure the health and safety of students so that they are ready to learn.
New students and students entering grades K, 1, 4, and 7 must submit a physical examination and immunization record before the first day of school. Exam reports should be dated no earlier than one year prior to the first day of school.
Immunizations are an important part of keeping our student population healthy. Immunization entrance requirements are set by the Massachsuetts Department of Public Health, and are listed below.
Students that do not meet these immunization requirements may be excluded from school. The only exception is for students with a medical or religious exemption that is documented by the child's pediatrician.
Please make sure to forward an updated copy of your child’s physical examination and immunization records to the school nurse after each annual visit. Health reports may be faxed to the attention of the school nurse at fax # 508-845-1535.
Health screenings are conducted by the nurse’s office each year. These screenings help the nurse to identify those students who meet criteria that may impair their ability to function at their optimum level in the school environment. The following health screenings are conducted every year:
Students who are ill during the night or in the morning should always be kept home. A child with vomiting and/or diarrhea must be symptom free for 24 hours prior to returning to school. If your child has a fever of 100.4 degrees Fahrenheit or higher, he or she must be fever free without the use of fever-reducing medication for 24 hours before returning to school.
Please call the school office at 508-842-1601 before 8:30am to report an absence. We will need the following information:
The office answering machine is on 24/7. You may leave a message to report an absence at any time of the night or day. Absences due to illness must be reported on a daily basis. Thank you for your cooperation!
Our medication policy complies with state guidelines for students who require medication during school hours. All medications must be kept in the nurse’s office.
LIFE-THREATENING ALLERGIES:
Many children have food and other serious allergies. If your child has a life-threatening allergy that requires an Epi-pen, please make sure to bring your child's medication to school along with a physician’s order (and/or an allergy action plan), and a signed parental consent form. Thank you.
AT THE END OF THE YEAR:
Please remember to pick-up all medications by the end of the school year. Medications must be picked up by a parent or guardian. Any medications left at school at the end of the school year will be discarded.
